Age-friendly care is health care that addresses your unique needs and wants. It can help you enjoy a better quality of life.
It is care that is safe and based on what research shows are the most important things to pay attention to as we get older - the 4Ms: what Matters, Medication, Mentation and Mobility.
Know and align care with each older adult's specific health outcome goals and care preferences including, but not limited to, end-of-life care and across settings of care.
If medication is necessary, use Age-Friendly medication that does not interfere with What Matters to the older adult, Mobility, or Mentation across settings of care.
Prevent, identify, treat, and manage dementia, depression, and delirium across settings of care.
Ensure that older adults move safely every day in order to maintain function and do What Matters.
Dr. Michael Bernardo opened Bernardo Hospice Care, bringing together a new team with many years of health care experience. These caring, committed, and highly qualified professionals are people Dr. Bernardo has known personally and worked alongside with both in hospice and long-term care. Together we will strive to bring the best hospice services to our patients and their loved ones. We will walk with you providing care, comfort, and the hope of peace throughout this stage of life’s journey.
At Bernardo Hospice Care, we believe that hospice recognizes dying as a process and we turn our focus to providing comfort rather than care. By providing physical, emotional, and spiritual care to terminally ill persons and their families, hospice neither hastens nor postpones death. We help those with a life limiting illness to maintain their quality of life and support their family members through this difficult last stage of life. We believe in a comprehensive approach of Care, Comfort, and Completion.
Hospice services are covered under several different insurance programs including Medicare, Veterans Administration, selected private insurance plans and some state funded programs. Your hospice benefit covers several medications, medical supplies, and medical equipment.
